A health-awareness video circulating online has drawn attention to a less visible danger associated with electronic cigarettes: the chemicals used to create attractive flavours may not be harmless when they are heated and inhaled.
The video, which features a health commentator explaining the difference between eating flavouring substances and inhaling them, highlights popcorn, fruit and cinnamon flavours and warns that some flavouring chemicals can irritate and damage lung tissue.
Scientific evidence broadly supports the central warning, although some of the claims require important qualification: a chemical being safe for use in food does not automatically mean it is safe to inhale, and evidence from laboratory studies does not necessarily mean that every flavoured vape will cause the same degree of injury in every user.
The World Health Organization (WHO) says e-cigarettes heat liquids containing nicotine and other additives, flavours and chemicals to produce an aerosol that is inhaled directly into the lungs. WHO states that these products are harmful to health and that some of the chemicals generated are known to be toxic.
The Problem With “Food-Safe” Flavours
One of the key messages in the video is that flavouring chemicals intended for consumption should not automatically be considered safe for inhalation.
That distinction is scientifically important.
The human digestive system and liver process substances that are swallowed, whereas vaping delivers an aerosol directly to the respiratory system. The lungs therefore become a primary site of exposure.
WHO has specifically identified flavouring compounds such as diacetyl, cinnamaldehyde and benzaldehyde as substances of concern when heated and inhaled.
The fact that a flavouring is permitted in food therefore does not establish that it is safe when repeatedly inhaled.
The “Popcorn Lung” Connection
The video refers to flavouring chemicals associated with popcorn.
There is a genuine scientific history behind this warning.
Diacetyl, a chemical historically used to create a buttery flavour, became strongly associated with severe occupational lung disease after workers in microwave-popcorn and flavouring-production facilities were exposed to high concentrations of its vapour.
The US National Institute for Occupational Safety and Health (NIOSH) documented cases of severe obstructive lung disease, including bronchiolitis obliterans, among workers exposed to diacetyl-containing flavourings.
The condition became popularly known as “popcorn lung.”
However, it is important not to overstate the evidence. The classic occupational cases involved substantial workplace exposure to flavouring chemicals. They do not prove that every person who uses a flavoured vape will develop bronchiolitis obliterans.
What they do demonstrate is that certain flavouring chemicals can cause serious respiratory injury when inhaled under sufficiently high or repeated exposure.
What Happens When Flavouring Chemicals Are Heated?
Another important point raised by the video is heating.
An e-cigarette does not simply release the flavouring unchanged. The liquid is heated to produce an aerosol, and the chemicals present can undergo thermal reactions.
WHO identifies carbonyl compounds such as formaldehyde, acetaldehyde and acrolein among substances of concern in e-cigarette emissions. Acrolein is a strong respiratory irritant, while formaldehyde is a known human carcinogen.
The amount and composition of chemicals produced can vary according to the liquid formulation, device, power setting, temperature and vaping behaviour.
This means that “it is only vapour” is a misleading description.
The user is inhaling an aerosol containing a mixture of chemicals and ultrafine particles.
Cinnamon Flavour Raises Particular Concerns
The video singles out cinnamon flavouring, and this is an area where laboratory research provides particularly interesting evidence.
A study published in Scientific Reports examined cinnamon-flavoured e-cigarette aerosol and found evidence of oxidative stress and oxidative DNA damage in exposed human bronchial epithelial cells. The cinnamon aerosol produced greater oxidative effects than the butter-flavoured aerosol tested in that experiment.
Another study examining cinnamaldehyde, the major chemical associated with cinnamon flavour, found that certain cinnamon-containing refill fluids and aerosols were toxic to human lung cells in laboratory experiments. The researchers also observed DNA strand breaks and other cellular effects.
A systematic review of research into e-liquid flavours similarly found evidence of inflammatory and toxic effects, while cautioning that the precise risks of many individual flavouring chemicals remain uncertain and that laboratory concentrations do not always correspond directly to human exposure.
DNA Damage Does Not Automatically Mean Cancer
This distinction is crucial when communicating health warnings.
The video refers to DNA damage caused by some flavouring aerosols. Laboratory studies have indeed detected oxidative DNA damage and other cellular changes following exposure to some flavoured e-cigarette aerosols.
But DNA damage in laboratory cells is not the same thing as proof that vaping causes cancer in humans.
DNA damage is one biological mechanism that can contribute to disease, but establishing a cancer risk requires evidence from human epidemiological studies and long-term exposure data.
Journalistic health reporting should therefore describe these findings as a warning signal, rather than claiming that a particular flavour inevitably causes cancer.
The Lungs Are Particularly Vulnerable
The respiratory system is designed to exchange gases while protecting the body from particles and harmful substances in the air.
Repeated exposure to irritants can trigger inflammation and oxidative stress.
Research on flavouring chemicals has found effects including increased inflammatory signalling, oxidative stress, impaired epithelial-barrier function and cellular toxicity in laboratory models.
Some flavouring chemicals can also interfere with normal respiratory defence mechanisms.
This is especially concerning because vaping can involve repeated exposure throughout the day, sometimes for months or years.
Flavours Can Make Vaping More Appealing
The danger is not confined to chemistry.
Flavours can also influence behaviour.
Fruit, candy, mint, dessert and other flavours can make nicotine products more attractive, particularly to young people.
WHO warned in 2025 that flavours in tobacco and nicotine products can mask the harshness of nicotine and encourage uptake among young people. The organisation has called for stronger action against flavoured nicotine products.
Nicotine itself is highly addictive, and WHO says people who use e-cigarettes can become dependent on it.
What About People Who Do Not Vape Nicotine?
The absence of nicotine does not necessarily make a vaping aerosol harmless.
Some electronic nicotine delivery systems and related devices can contain nicotine-free liquids, but the aerosol can still contain flavouring chemicals and other potentially toxic substances.
WHO notes that e-cigarette liquids may or may not contain nicotine but can contain additives, flavours and chemicals harmful to health.
Therefore, the argument that a product is “safe because it has no nicotine” does not address the potential respiratory effects of inhaled flavouring chemicals.
Uganda Already Has a Legal Warning
For Ugandans, the issue has an additional dimension.
Uganda’s Tobacco Control Act, 2015 prohibits the importation, manufacture, distribution, processing, sale, offer for sale or bringing into the country of electronic nicotine delivery systems, including electronic vaporisation devices and nicotine-containing cartridges or other substances intended to be vaporised. The law also addresses waterpipe tobacco systems and certain flavoured tobacco products.
WHO has previously described Uganda as having a ban on e-cigarettes and has urged stronger enforcement of tobacco-control measures.
The WHO’s 2023 Uganda country profile likewise recorded restrictions on electronic nicotine and non-nicotine delivery systems.
The issue therefore goes beyond an internet health trend: it also concerns compliance with Uganda’s public-health and tobacco-control framework.
Parents Should Pay Attention
The appealing appearance and flavours of vaping products can make them difficult for parents to recognise as nicotine products.
Parents, teachers and guardians should pay attention to signs such as unfamiliar vaping devices, sweet or unusual odours, unexplained coughing or throat irritation, and changes in behaviour associated with nicotine dependence.
WHO has repeatedly expressed concern about the marketing and uptake of e-cigarettes among children and adolescents.
Young people are particularly vulnerable because nicotine exposure during adolescence can affect the developing brain and contribute to addiction.
The Bottom Line
The strongest scientific message from the video is not that every flavoured vape will cause “popcorn lung.”
Rather, it is this:
Flavouring chemicals should not be assumed to be safe simply because they are safe to eat.
There is credible evidence that some flavouring compounds can irritate the respiratory tract, generate toxic substances when heated, promote oxidative stress and inflammation, and cause cellular damage in laboratory studies. WHO considers e-cigarettes harmful and continues to call for stronger regulation.
For people who do not smoke, the safest choice is not to start vaping.
For people already dependent on nicotine, quitting remains the healthiest option. Those experiencing persistent cough, chest pain, shortness of breath, wheezing or other respiratory symptoms should seek assessment from a qualified health professional rather than assuming the symptoms are harmless effects of vaping.
